Applying for CareCredit is quick and easy simply choose from the following three options: 

1. STANDARD APPLICATION:  Complete and sign application in doctor’s office.

2. ONLINE APPLICATION:  Go to www.carecredit.com  Fill out patient application and receive a decision immediately.

3. AUTOMATED PHONE APPLICATION:  Call 800-365-8295 and follow prompts and receive a decision immediately.
